Our vision is to continuously improve the health and wellbeing of the people of Southern Derbyshire, using all resources as fairly as possible

We will do this by:

  • providing local clinical leadership to the NHS, and working with everybody who can contribute to our aims;

  • being open and accountable to our patients and communities; ensuring they are at the heart of everything we do;

  • understanding our population and addressing inequalities so that services are in place to meet needs;

  • planning services that best meet those needs now and in the future;

  • aiming to secure the best quality, best value health and social care services we can afford;

  • using our resources fairly and effectively.

Our Corporate Objectives 

1. Maintain compliance with, and delivery of, ‘must do’ tasks

  • Meet NHS Constitution standards on access, referral, and treatment targets

  • Meet NHS Constitution standards for cancer care, and implement key recommendations of cancer strategy

  • Meet our statutory responsibilities for Safeguarding Children and Adults

  • Meet our statutory duties on provision of Children’s and Young People’s services  

2. Improve services to make them fit for the future  

  • Transform GP services to improve access, retain workforce, maximise technology, redistribute workload and improve quality

  • Transform diabetes services to improve education and long-term outcomes for patients

  • Transform urgent/on-the-day care services to reduce pressure and ensure patients are seen in the right place at the right time

  • Transform elective care services to reduce workload and deliver a smoother patient journey

  • Transform learning disability services to improve access, community care and life expectancy

  • Transform mental health services to ensure timely access to diagnosis and treatments

  • Improve ‘proactive’ and ‘reactive’ support for patients to help avoid hospital admissions

  • Implement ‘place-based’ commissioning

  • Improve prescribing and compliance of medications

  • Continue to deliver other identified priorities as part of the Derbyshire STP  

3. To achieve financial balance for the CCG as part of a wider health and social care system

  • Deliver against our financial targets

  • Identify areas for saving money and implement them, including identified ‘Rightcare’ areas

  • Combine CCG functions across Derbyshire
