Can you please provide me with a copy of the most recent guidelines for vitamin D deficiency, insufficiency and maintenance for Southern Derbyshire CCG as well as a copy of any patient information leaflets for these conditions.

Also please provide any recommendations to prescribers or patients regarding any vitamin D supplements that are recommended to purchase over-the-counter (OTC). Please include CCGs and other organisations which adhere to these guidelines.

  • How many cycles of IVF do you offer to eligible patients?
  • Do you fund a full cycle of IVF, as defined by NICE, which includes replacement of all frozen embryos? If not, how many frozen embryo transfers do you fund?
