The Health Panel is a group of interested people who register their details with the CCG. It enables us to contact members of the public so that they can be involved in the decisions that NHS Southern Derbyshire Clinical Commissioning Group makes.
There are varying levels of involvement in the Health Panel.
- You can receive regular updates about what we are doing and you can offer us feedback
- You can act as a reader for us and proof read patient communications and information
- You can come along to events that we or our partner health or social care organisations run
- You can work with us on learning more about what we do and then get involved in projects working with us
To help us understand more about who you are and what you are interested in we will ask you to complete a registration form. This will help us to know how you would like to be involved and if there is anything in particular that interests you.
We do ask for a range of information including your gender, age and sexuality. This information allows us to understand more about our Health Panel as a collective and where we need to do some more targeted work to ensure our Health Panel is reflective of our communities.
Once you have signed up you will receive all information related to the Health Panel as routine and this will usually be over e-mail. If you do not have access to e-mail then we can find an alternative that meets your needs.
Your level of involvement is completely up to you and any support you can give us will be appreciated.
If you would like to join our Health Panel then please complete both forms or apply online:
You can email the forms back to us, post them for free using the address below or contact us directly:
