We’re teaming up with staff from Royal Derby Hospital, Derby City Council and voluntary sector partners to promote a personal profile which supports people with dementia.
The ‘All About Me’ document keeps important information in one place to help those providing care and support to people with dementia to understand their needs and preferences so they get the best possible care.
Members of the Southern Derbyshire Dementia Implementation Group is hosting displays tomorrow (22 July) in the foyer at The Council House (Derby City Council) between noon and 3pm and at Royal Derby Hospital all day.
People with dementia and memory loss and their carers will be encouraged to complete the ‘All About Me’ form so they are ready if they have to be admitted to hospital or receive any other support service.
