NHS organisations and local councils have come together to develop plans for the future of health and social care. Derbyshire’s Sustainability and Transformation Plan (STP), Joined Up Care Derbyshire, brings together work that has been taking place across the county to coordinate services better to support people in staying well.
All 11 NHS and social care partner organisations working on Joined Up Care Derbyshire will look at improving how they work together, supporting people who are most at risk and making sure health and social care professionals get the best opportunities to help people lead happier, healthier lives.
Services will focus on looking after people in their home or local area, so they get the targeted care they need to get better more quickly, instead of concentrating on providing services from specific buildings.
Joining up care is also about preventing people from getting ill in the first place by helping them take good care of themselves and be able to deal with issues before they become bigger problems.

Practice Manager Representation Needed… Derbyshire GPFV Workforce Steering Group
Derbyshire’s General Practice Forward View (GPFV) Workforce Steering Group is currently seeking Practice Manager representation.
The purpose of the Derbyshire General Practice Workforce Steering Group is:
- To lead and coordinate the implementation of the GPFV Workforce Plan and any workforce implications arising from the Access to Primary Care workstream in Joined Up Care Derbyshire
- To ensure that the impact of and interdependency with national and local transformation and service plans on the General Practice Workforce in Derbyshire are understood and considered by the Derbyshire Health and Social Care System
