If you have Type 2 diabetes and want to learn how to manage your condition better, you can ask your GP or Practice Nurse to refer you onto a FREE 6-week education programme to learn more about the condition and how to manage it better yourself.
The course, called ‘X-PERT Health Diabetes Programme’, is award winning and delivers structured education over 6 weeks in sessions of 2½ hours per week. X-PERT stands for ‘Expert Education versus Routine Treatment’. You’ll attend the course along with other people who also want to do the same.

We can now confirm that Freestyle Libre is available from specialist diabetes services in Derbyshire for patients who meet the eligibility criteria as determined by the Derbyshire Joint Area Prescribing Committee (JAPC). If you have any questions about your suitability then please ask your Diabetic Specialist Nurse or Consultant at your next routine appointment.
