Why Place-based Care?
City Centre South has been working as a Place for just over a year. Meetings are held on a monthly basis and alternate between practices. The meetings are attended by practice representatives, namely Lead Clinicians and Practice Managers and wider Stakeholders, including DCHS, Public Health and Adult Social Care.

As a Place, the vision is to provide excellent quality of patient care, close to the patient’s place of residence and to proactively support the health and wellbeing of our local population.

City Centre South has been selected to participate in a Diabetes PCES (Proof of Concept Enhanced Service). This service will support and improve the care and outcomes for people with diabetes delivered locally.

Clinical Leadership
To support the development of Place-based Care in City south, Dr Mahya Johnson has been identified as Clinical Lead.

Working in partnership across practices, organisations that impact on health and wellbeing, and with local patients and community groups, the aim of the role is to provide leadership and agree a joint vision and purpose for the ‘Place’. 

City Northeast and City Centre South Place Development
City Northeast and City Centre South Places are working together on a number of projects including: 

  • Extending access to primary care services for the local population
  • Providing locally based smoking cessation and weight loss support services
  • Exploring the opportunity to work with other providers to enhance home visiting services for housebound patients and care home patients
