What We Have Done

Eight-bed Intermediate Care Centre 

NHS Southern Derbyshire Clinical Commissioning Group (CCG) has opened an eight-bed intermediate care centre in the Oakland Village for older people in Swadlincote.   

The service covers the following South Derbyshire General Practices: 

  • Heartwood Medical Practice

  • Gresleydale Healthcare Centre

  • Newhall Surgery

  • Swadlincote  Surgery

  • Overseal Surgery

  • Woodville Surgery 

The team provides therapy and holistic care in conjunction with social care staff for up to 6 weeks of rehabilitation and it is envisaged that the beds would be utilised in a combination of ‘step up and step down’ admissions, being used flexibly to meet  individual needs in collaboration with Health and Social Care within the South Derbyshire area. 

Single Point of Access (SPA) 

Swadlincote Health Centre has a SPA (Single Point of Access) staffed by an administrator and navigator (currently a qualified nurse or physiotherapist). The Navigator carries out an initial triage of the referral and passes it on to the appropriate therapists. 

Referrals are made to the SPA from:

  • GPs

  • Community staff

  • Hospital staff (discharge coordinator/therapists)

  • Social Services 

Single Point of Access Service Achievements to Date:

  1. Responded same day or within 24 hours to referrals, working effectively with new SPA navigation service based at Swadlincote Health Centre.

  2. Partnership working with Social Care to ensure efficient and effective day to day working on the unit and provision of high quality care.

  3. Provided value for money by preventing unnecessary hospital admissions.

  4. Kept patients at home for longer.

  5. Prevented early admission into care homes.

  6. Signposted or referred to other agencies if patient not suitable for admission - liaising with social care/intermediate care/domiciliary therapy services/community matron.

  7. Management of  complex patients(service users)  during a time of change - giving time to explore their long term needs and options. 

The SPA is informed daily about the bed status of the Intermediate Care Centre.
