NHS North Derbyshire Clinical Commissioning Group (CCG), on behalf of NHS Southern Derbyshire CCG and others, is inviting applications from voluntary sector organisations to provide high quality supportive services.
After an extensive piece of work earlier this year to strengthen the way health and social care funding is distributed by the public sector to the voluntary and community sector, our CCG has identified the following priorities which are important when awarding funding. These are:
- Early intervention, self-help and prevention
- Prevention of hospitalisation and support for discharge
- Reducing social isolation
Organisations applying for funding must clearly demonstrate how their service meets the main priorities as identified above and is in line with the strategic areas of work as set out by Derbyshire’s Health and Wellbeing Board.

Value of contract
Funding will be awarded at a rate of £2,000 - £50,000
- BravoSolution is open for applications until 12 noon on Friday 19 August 2016.
- Contracts will be awarded from 1 October 2016 onwards.
